Background: Emotion dysregulation is a core feature of bipolar disorder (BD), yet its behavioral expression during depressive episodes, and potential differences between its types, BD-I and BD-II, remain unclear. This study used automated facial-expression analysis during naturalistic affective film viewing to examine subtype-specific and context-dependent emotional responding in bipolar depression. Methods: The sample included 135 participants: 69 healthy controls and 66 patients with BD (BD-I, 23; BD-II, 43). Participants viewed nine emotionally evocative film clips spanning negative, positive, neutral, and socially threatening contexts, while their facial expressions were continuously recorded and quantified using computer vision-based facial-expression analysis. Results: Patients with BD-I showed a distinct, context-dependent facial-expression profile, characterized by greater negative responses across multiple contexts than other groups. Specifically, they showed increased sadness during sad, reward, and amusing clips, and elevated anger during sad and neutral clips. In socially threatening contexts, BD-I participants showed a multivalent pattern of elevated anger, fear, and joy, suggesting poorly coordinated or context-incongruent affective expression. In contrast, BD-II participants did not differ significantly from healthy controls on any emotion, despite depressive symptom severity comparable to BD-I participants. Conclusions: These findings suggest that facial-expression patterns in bipolar depression differ across subtypes. BD-I may be characterized by heightened negative reactivity and altered context-appropriate modulation of emotional expression, whereas BD-II may not show comparable alterations in overt facial output. Automated facial-expression analysis during naturalistic stimulation may provide a useful behavioral marker for characterizing subtype-specific affective disturbance in bipolar depression and related psychopathology.

Purpose This study aimed to examine the association between screen time and depressive symptoms among Chinese college students, and to investigate the mediating roles of sleep quality and emotion regulation in this relationship. Furthermore, a serial mediation model was constructed to elucidate the underlying psychological mechanisms linking screen exposure to depression. Methods A stratified cluster sampling method was employed to recruit 10,999 college students for a cross-sectional questionnaire survey. Data were collected on screen time, sleep quality, emotion regulation ability, and depressive symptoms. Descriptive statistics, correlation analyses, and regression analyses were conducted using SPSS 26.0 A serial mediation model was tested using the PROCESS macro (Model 6), and bootstrapping procedures were applied to estimate the significance of indirect effects. Results Correlation analyses indicated that screen time was significantly positively associated with depressive symptoms (r = 0.16, p < 0.01) and sleep quality (r = 0.15, p < 0.01), and significantly negatively associated with emotion regulation (r = -0.13, p < 0.01). Sleep quality was positively correlated with depressive symptoms (r = 0.31, p < 0.01), whereas emotion regulation was negatively correlated with depressive symptoms (r = -0.42, p < 0.01). Regression analyses further showed that screen time significantly positively predicted depressive symptoms ({beta} = 0.712, p < 0.001), positively predicted sleep quality ({beta} = 0.217, p < 0.001), and negatively predicted emotion regulation ({beta} = -0.085, p < 0.001). In addition, both sleep quality ({beta} = 1.318, p < 0.001) and emotion regulation ({beta} = -0.424, p < 0.001) were significant predictors of depressive symptoms. Mediation analyses demonstrated that sleep quality significantly mediated the association between screen time and depressive symptoms (95% CI [0.239, 0.332]), as did emotion regulation (95% CI [0.269, 0.416]). Moreover, a significant serial mediation effect of sleep quality and emotion regulation was observed in the relationship between screen time and depressive symptoms (95% CI [0.082, 0.117]). Conclusion Screen time is significantly associated with depressive symptoms among college students, with sleep quality and emotion regulation serving as important mediating mechanisms. Extended screen exposure may be linked to higher levels of depressive symptoms by impairing sleep quality and weakening emotion regulation capacity.

Background Functional neurological disorder (FND), one of the most common neurological conditions, affects women almost twice as frequently as men. The reasons for this are unknown, and there has been minimal research into how physiological and pathological features of women's health interact with symptoms of FND. Methods We conducted an online survey assessing the effect of several aspects of women's health with the severity of symptoms of FND. Results 484 people completed the survey. Among the 223 who had regular or fairly regular menstrual cycles, a strong difference across the menstrual cycle was seen, with symptoms at their best in the follicular phase, worsening in the luteal phase, and worst in the pre-menstrual period and the menses. This effect was not moderated by a proxy measure of pre-menstrual dysphoric disorder (PMDD). Participants who were taking the combined oral contraceptive (COC, n=43) and progesterone-based contraception (n=80) were more likely to report symptom improvement from starting the medication than worsening. When compared to menstruating participants who were not taking the COC, participants taking the COC reported less worsening in their symptoms of FND in the luteal, pre-menstrual, and menstrual phases. Of the 99 women who had passed menopause since developing FND, 76% reported worsening of their FND symptoms after menopause. Discussion This study demonstrates interactions between several aspects of women's health and symptoms of FND. The observed pattern of symptom fluctuation across hormonal states suggests a potential modulatory role of oestrogen, warranting further targeted investigation.

Purpose: Suicide mortality is underestimated due to misclassification under undetermined and accidental deaths. This study examined national trends in suicide and related external causes of death in Portugal from 2002 to 2023, by sex and age group, assessing potential shifts suggesting masked suicide and quantifying the relationship between undetermined, suicide, and accident death rates through ratio indices. Methods: Using official mortality data from Portugal's Statistics Institute (INE) for 2002-2023, we calculated age-standardised (SDR) and age-specific death rates (ASDR) for suicide (X60-X84), undetermined intent deaths (Y10-Y34), and unintentional deaths (V01-X59), disaggregated by sex and four age groups (15-24, 25-44, 45-64, 65+). We estimated undetermined-to-suicide (UnD:Suic) and undetermined-to-accidents (UnD:Accs) rate ratios for SDRs and ASDRs. Trends were analysed using joinpoint regression (APC/AAPC) and structural breakpoint analysis (Chow test, BIC). Results: Suicide SDRs declined across the period for males (AAPC: -2.25%) and females (AAPC: -1.32%), with the sharpest reductions among males aged 25-44 (AAPC: -2.56%) and females aged 65+ (AAPC: -2.44%). Deaths of undetermined intent rose steeply from 2002 to 2005-2006 and declined thereafter. Unintentional deaths declined in most age groups, except females aged 65+ (AAPC: +1.41%). Both ratio series peaked around 2005-2009, declined progressively through the 2010s, and reached their lowest values in 2021-2022. Age-specific analyses revealed a significant and sustained increase in both ratios among females aged 45-64. Structural breakpoints clustered around 2004, 2013-2015, and 2019-2020. Conclusion: Suicide mortality declined in Portugal from 2002 to 2023, but divergent trends in undetermined and accidental deaths across sex and age subgroups highlight ongoing misclassification. Age- and sex-specific ratio analyses identify the population subgroups where misclassification is most concentrated, providing a foundation for future imputation-based estimates of probable suicide burden.

Background: Depression is clinically heterogeneous, and immune-metabolic disturbances may not map uniformly onto categorical diagnosis or symptom severity. In populations exposed to substantial structural adversity, it remains unclear whether inflammatory and metabolic biomarker differences reflect adversity exposure itself or cluster around specific depressive phenotypes. Methods: Data were drawn from the Sao Paulo Megacity Mental Health Survey, a population-based study in which 5,037 household residents underwent structured psychiatric interviews. Among 770 participants assessed as having clinically significant symptoms (SCID-I), individuals with chronic physical illnesses, hs-CRP >20 mg/L, or missing data were excluded, yielding an analytic sample of 653. Latent class analysis of 16 DSM-IV depressive symptoms was used to identify symptom-derived phenotypes. Multinomial logistic regression tested associations between latent classes and immune-metabolic biomarkers, including hs-CRP, lipid fractions, fasting glucose, and triglycerides, adjusting for age, sex, education, smoking, and BMI. Results: A four-class solution identified asymptomatic (44.56%), mild-moderate (19.14%), atypical-like (16.69%), and melancholic-like (19.60%) classes. The two highseverity classes diverged by neurovegetative features: atypical-like depression was characterised by weight gain, hypersomnia, and psychomotor retardation, whereas melancholic-like depression was characterised by weight loss, insomnia, and psychomotor agitation. hs-CRP was highest in the atypical-like class and lowest in the melancholic-like class despite similar symptom severity. After BMI adjustment, elevated hs-CRP in the atypical-like class attenuated, whereas lower hs-CRP in the melancholic-like class persisted. Other metabolic markers did not robustly differentiate classes after adjustment. Conclusions: Immune-metabolic differences in depression do not simply follow symptom severity. In this Sao Paulo cohort, higher and lower hs-CRP profiles clustered around distinct latent depressive phenotypes, supporting biologically heterogeneous depressive presentations within a socially exposed urban population.

This qualitative study evaluated the Life Skills (HpV) training strategy at a public university in Colombia in 2024, analyzing its impact on students positive mental health and psychosocial competencies. The mixed-methods research employed semi-structured interviews with faculty and three student focus groups, using thematic analysis to categorize strengths, weaknesses, and perceived changes. Results highlighted curricular coherence, academic freedom, and participatory methodology as key aspects, fostering self-awareness, emotional management, and the building of support networks. Students reported improvements in well-being, stress management, and academic performance, though challenges such as initial resistance to emotional content and student diversity were identified. The conclusions underscore the value of experiential courses in university education, promoting horizontal relationships and safe spaces for collective reflection. Future studies are recommended to expand participant diversity and incorporate quantitative data triangulation to further explore the interventions effects.

Introduction Auto-Induced Cognitive Trance (AICT) is a non-ordinary state of consciousness (NOSC) that can be accessed by will alone once a standardised self-induction procedure has been learnt. The first research publication on AICT dates back only ten years, meaning that research on this phenomenon is still in its infancy. Previous reports concerning the phenomenology and neurophysiology of AICT revealed similarities with more extensively described NOSCs, as well as unusual features, raising questions about the potential benefits of AICT practice for well-being. Objective This study aimed to gather quantitative descriptive data on features associated with well-being in a large comparative sample of AICT practitioners and non-practitioners. Method This research followed a web-based survey study design which enquired AICT-trained and yet-to-be trained participants to self-report through validated standardised questionnaires on vitality, self-esteem, mental well-being, trait anxiety, life satisfaction, happiness, positive and negative affect, nature-relatedness and connectedness. Data on NOSCs practices, life history events that could have led to spontaneous NOSCs, and demographic data were collected for further inclusion as control variables in statistical models. Results The online questionnaire yielded 607 valid responses, (171 yet-to-be trained participants and 436 AICT-trained participants). AICT practice was found to be associated with increased self-esteem (RSE), overall connectedness (WCS) as well as all subdimensions of connectedness (WCS Self, WCS Others, WCS World). AICT practice Duration exhibited significant effects on global connectedness and all subdimensions of connectedness, self-esteem, trait anxiety (STAIT-5), and positive affect (PANAS+). Conclusions AICT seems to benefit to practitioners well-being shortly after training through increases in self-esteem and in the sense of connectedness. Prolonged AICT practice is associated with added decreased trait anxiety and increased positive affect. Further research is needed to confirm these findings with a sample including AICT-uninterested participants, and to clarify the underlying mechanisms of AICT.

Background Oral care is a critical nursing intervention for mechanically ventilated patients in intensive care units (ICUs) and plays an important role in preventing ventilator-associated complications. However, variability in nurses' competency in oral care remains a concern, particularly in resource-limited settings. Objective To assess ICU nurses' competency in providing oral care to mechanically ventilated patients and determine factors associated with competency at Tenwek Hospital, Kenya. Methods An analytical cross-sectional study was conducted among ICU nurses at Tenwek Hospital. A total of 38 nurses were invited, and 35 participated, yielding a response rate of 92.1%. Data were collected using a structured questionnaire and an observational competency checklist. Descriptive statistics and inferential analysis, including chi-square tests and binary logistic regression, were performed using SPSS version 30. Statistical significance was set at p<0.05. Results ICU nurses demonstrated generally high competency in key oral care practices, including use of personal protective equipment (100%), suctioning before and after oral care (91.4%), and oral assessment (80.0%). However, gaps were identified in documentation of oral care (62.9%) and adherence to standardized protocols (65.7%). Formal oral care training was significantly associated with competency (OR=5.63, 95% CI: 1.78-17.81, p=0.002), as were professional qualification (p=0.030) and ICU experience (p=0.021). In multivariable analysis, oral care training (OR=3.01, p=0.006), ICU experience (OR=2.85, p=0.015), and availability of guidelines (OR=2.17, p=0.047) were independent predictors of competency. Conclusion ICU nurses at Tenwek Hospital demonstrate satisfactory competency in oral care for mechanically ventilated patients, although gaps remain in documentation and protocol adherence. Strengthening training, guideline availability, and institutional support systems is essential to improve consistency and quality of oral care practice. Background: Cardiovascular disease (CVD) is the leading cause of mortality in low- and middle-income countries (LMICs). In Haiti, depression remains an underexplored CVD risk factor. We assessed the association between depressive symptoms (DS) and prevalent CVD in urban Haiti and examined sex differences. Methods: We conducted a cross-sectional analysis of enrollment data from the Haiti Cardiovascular Disease Cohort (adults [≥]18 years; March 2019--August 2021). DS were measured using the Patient Health Questionnaire-9 (PHQ-9) and categorized as none--mild (<10) versus moderate--severe ([≥]10). Prevalent CVD (angina, myocardial infarction, transient ischemic attack or stroke, heart failure) was adjudicated using epidemiologic definitions aligned with international cohorts. We estimated prevalence ratios (PRs) using generalized estimating equation Poisson models with a log link, adjusting for age, sex, education, income, food insecurity, smoking, alcohol use, physical activity, stress, and BMI. Effect modification by sex was assessed on multiplicative and additive scales. Results: Among 2,995 participants (mean age 41.9 years; 58.0% female), 16.2% (95% CI: 14.8-17.5; n=484) had moderate--severe DS. Prevalence was higher in females (22.0%, 95% CI: 19.8-23.6) than males (8.5%, 95% CI: 7.0-10.1). The prevalence of CVD was higher among participants with moderate--severe DS compared with those with none--mild DS, with similar patterns observed in both sexes (males: 21.5% vs 10.6%; females: 23.3% vs 15.3%). Moderate--severe DS were associated with higher CVD prevalence compared with none--mild DS (adjusted PR [aPR]=1.36; 95% CI: 1.08--1.71). In sex-stratified models, aPRs were 1.38 (95% CI: 1.06--1.78) for females and 1.25 (95% CI: 0.75--1.99) for males. Evidence for interaction by sex on the additive scale was limited (RERI=0.07, 95% CI: -0.74 to 0.88). Conclusion: Moderate--severe DS were independently associated with a higher prevalence of CVD in urban Haiti. Associations were consistently stronger among women, although evidence for effect modification by sex was limited. Integrating depression screening and management into CVD prevention efforts may help address the growing burden of both conditions in resource-limited settings. Prospective studies are warranted to better understand the underlying mechanisms and causal pathways.

Background: Childhood adversity (CA) has been identified as one of the most robust risk factors for psychotic disorders; several treatable mediating mechanisms have been proposed. Aims: To conduct a systematic review and meta-analysis examining mediating pathways linking CA and psychosis. Method: This PRISMA-compliant systematic review (PROSPERO: CRD42024542972). consisted of a search conducted in January 2026 on Ovid (PsycINFO, Medline, and Embase) using search terms related to psychosis, CA, and mediation analyses. Evidence was appraised by calculating the percentage of the total effect mediated in each study, grouping mediators into meaningful groups. When possible, meta-analyses using two-stage meta-analytic structural equation modelling (METASEM) were conducted. Results: 117 studies were included (54 in clinical samples, 59 in non-clinical samples, and four studies in both clinical and non-clinical samples). 107 studies examined psychological mediators and 12 examined biological. The median percentages of total effect mediated across all analyses per mediator family were: 49% for dissociation (k = 24), 45% for psychosocial stressors (k = 6), 37.9% for negative schemas (k = 23), 35.2% for post-traumatic symptoms (k = 10), 31.5% for depressive symptoms (k = 14), 27.8% for anxiety (k = 11), 27.1% for attachment styles (k = 12), and 8.7% for mentalization domains (k = 5). Meta-analyses confirmed a robust mediating effect of dissociation (k = 7; N = 2143; indirect effect (I.E) =0.42 [0.17, 0.66] on psychosis; 50.49%), on delusions (k =7; N = 1053; I.E = 0.36, [0.27, 0.46]; 46.44%]) and on hallucinations (k =10; N = 5705; I.E = 0.28 [0.20, 0.36]; 57.59%). Robust mediation via depression (k =5; N= 5028; indirect effect= 0.33 [0.31, 0.35]; 31.05%) and negative schemas of the association between trauma and psychosis broadly defined (k = 7; N=10791; I.E= 0.26 [0.17, 0.35]; 26.36%) was also observed. High heterogeneity was observed across all meta-analyses. Fewer studies examined biological mediators, preventing quantitative synthesis. Conclusions: Childhood adversity impacts psychosis through psychosocial mediators, particularly dissociation. Further work is required to on the potential role of biological mechanisms and its interplay with psychological mechanisms.

Background Brain abnormalities related to childhood adversity (CA) have been reported across clinical presentations in psychotic disorder (PD) and bipolar disorder (BD). This systematic review and meta-analysis examined gray matter volume (GMV) alterations linked to CA in PD and BD. Methods A PRISMA-compliant systematic review was conducted (PROSPERO ID: CRD42022351133). The EMBASE, MEDLINE, and PsycINFO databases were searched from inception to June 2024 for studies investigating CA and structural brain imaging in PD and BD. Study quality was assessed with the Newcastle Ottawa Scale (NOS). Data were extracted and synthesized accounting for sex differences and CA subtypes with brain findings categorized by the presence and direction of associations. Meta-analyses were performed for hippocampal and amygdala volumes. Results In the systematic review (k = 29), 3,056 participants with PD and BD (mean age = 36.6; SD =16.1; 47% female), published between 2011 and 2023, were included. Study quality was fair, with high heterogeneity. Most studies reported significant negative associations between CA and GMV, especially in prefrontal regions, while findings for the hippocampus and amygdala were largely null or inconsistent. Meta-analyses of a study subset identified no significant association between CA and hemisphere-specific and combined volumes of the hippocampus (k = 5; p [≥] 8805; 0.66) or amygdala (k = 4; p [≥] 8805; 0.87). Conclusion CA was not consistently associated with hippocampal or amygdala volume alterations in PD and BD. More consistent evidence emerged for reduced GMV in prefrontal regions, suggesting that neurobiological impact of CA may be more robustly captured at the cortical level.

Objective Psychosis spectrum symptoms (PSS) are common among children and youth. These symptoms may be clinically significant as studies indicate a heightened risk of mental health disorders, in general, as well as psychotic disorders, specifically, in youth that endorse PSS. This systematic review and meta-analysis investigates the longitudinal association between PSS in children and youth and subsequent mental health diagnosis. Methods A comprehensive search of Ovid Medline, PsycINFO, and EMBASE databases was conducted to identify longitudinal studies that: (i) assess PSS at a baseline timepoint, (ii) in individuals under 25 years, and (iii) assess mental health disorder diagnosis using a structured assessment at a later time point in the same sample. We conducted a meta-analysis and calculated pooled odds ratios (ORs) for mental health and psychotic disorders using random-effects models. Post-hoc meta-regressions were performed to examine the influence of a number of moderators on the relationship between earlier recorded PSS and subsequent mental health disorders or psychotic disorders. Results The search yielded 41 eligible studies of which 25 were included in the meta-analysis. Most included studies assessed PSS using brief self-report measures and recruited their samples from clinical or community settings. Among children and youth without an identified mental health diagnosis at baseline assessment, baseline PSS were associated with a 2-fold (OR = 2.07, CI = 1.61 - 2.66, I2 = 86.92%, p < 0.0001) increased risk of meeting diagnostic criteria for subsequent mental health disorder diagnosis and a 3-fold increased risk (OR = 3.11, CI = 2.11 - 4.58, (I2 = 60.93%, p < 0.0090) of meeting diagnostic criteria for a subsequent psychotic disorder diagnosis with a minimum 1 year follow-up time from baseline assessment. Meta-regression analysis indicated that study quality and sample size explained a substantial proportion of between-study heterogeneity for psychotic disorder outcomes. Conclusions Our results suggest that administration of simple self-report measures of PSS in both clinical and community settings may be helpful to identify children and youth at higher risk of subsequently meeting criteria for a mental disorder generally, and for a severe mental illness (i.e., psychotic disorder), specifically. Future longitudinal studies should focus on improving study design characteristics to increase confidence in identified longitudinal associations. The results of our work suggests that integration of self-report measures of PSS may be useful in a variety of settings to identify youth at increased risk of subsequent mental illness.

Aphasia diagnosis in Indonesia remains challenging due to limited culturally and linguistically appropriate instruments. Widely used tools such as the Boston Diagnostic Aphasia Examination (BDAE) and Western Aphasia Battery (WAB) are not adapted to the Indonesian context, while Tes Afasia untuk Diagnosis, Informasi, dan Rehabilitasi (TADIR) provides screening but lacks diagnostic accuracy. To address this gap, we developed the Instrumen Diagnosis dan Evaluasi Afasia (IDEA) for native Indonesian speakers and evaluated its validity, reliability, and normative cutoff values in cognitively healthy Indonesian adults. Eighty-three cognitively normal adults (screened using MoCA-Ina) with no history of neurological disease were assessed using IDEA, which evaluates six language domains. Items were adapted from existing tools and reviewed by experts. Content validity, internal consistency (Cronbachs alpha), and construct validity (Exploratory Factor Analysis) were analyzed using SPSS v25. A total of 83 participants were included (median age = 55.81 years, 54% secondary education). IDEA demonstrated good feasibility, with an average completion time of 45-60 minutes depending on participant engagement. Content validity was established by unanimous expert consensus. Construct validity showed meritorious sampling adequacy (KMO = .872) and significant sphericity (Bartletts test {chi}^2 (15) = 278.523, p<.001), supporting factor analysis. Internal consistency showed good reliability across six domains (Cronbachs = 0.896). IDEA is a valid and reliable tool for assessing aphasia in Indonesian natives. It is a culturally appropriate assessment tool which offers structured, domain-based evaluation and supports differential diagnosis of both classical and progressive aphasia syndromes. ABSTRACT Objective Delayed cerebral infarction (DCIn) is a severe complication following aneurysmal subarachnoid hemorrhage (aSAH). Previous studies suggest that glycemic variability is associated with DCIn. However, whether diabetes status modifies the relationship between glycemic traits and DCIn remains unknown. Methods Clinical data were collected from aSAH patients admitted to the First Affiliated Hospital of Shantou University Medical College between January 2015 and April 2025. The collected data included demographic characteristics, clinical variables, and glycemic traits. Glycemic traits included mean blood glucose (GLU-M), standard deviation of blood glucose (GLU-SD), coefficient of variation of blood glucose (GLU-CV), variance of blood glucose (GLU-Var), range of blood glucose (GLU-R), average real variability of blood glucose (GLU-ARV), and variability independent of the mean (GLU-VIM). After 1:2 case-control matching, conditional logistic regression models were used to evaluate the associations between glycemic traits and DCIn risk, with stratified analyses performed according to diabetes status. Multiplicative interaction terms were additionally included to assess the potential modifying effect of diabetes status. Results A total of 306 patients with aSAH were included. Among them, 102 developed DCIn cases. For each of these 102 cases, two controls were matched by age ({+/-}5 years), sex and year of admission ({+/-}5 years). In the overall population, higher GLU-M and GLU-ARV were associated with increased DCIn risk, with odds ratios (ORs) per 1-SD increase of 1.62 (95% CI, 1.25-2.11) and 1.63 (95% CI, 1.25-2.11), respectively. Among patients without diabetes (n=266), the associations with DCIn per 1-SD were observed for GLU-M (OR, 2.23; 95% CI, 1.56-3.19), GLU-SD (OR, 1.53; 95% CI, 1.13-2.06), GLU-Var (OR, 1.48; 95% CI, 1.04-2.10), and GLU-ARV (OR, 1.88; 95% CI, 1.38-2.55). No significant associations were observed among patients with diabetes. Significant interactions were observed between diabetes status and GLU-SD and GLU-Var, with P for interaction values of 0.033 and 0.032, respectively. Conclusion Higher mean blood glucose and greater glycemic variability are associated with an increased risk of DCIn in aSAH patients, especially in those without diabetes.

Background: Actigraphy-derived rest-activity rhythm (RAR) features are widely used to characterize clinical states in bipolar disorder (BD). Both mean levels and temporal variability of these features have been associated with mood episodes; however, variability measures are often statistically coupled with the mean, particularly in skewed distributions. This raises a question as to whether variability reflects a separate characteristic of the data or whether the observed association arises from statistical properties of the data. Objective: In this study, we aim to determine whether temporal variability of actigraphy-derived RAR features provides standalone information on mood episodes in BD beyond mean activity levels after accounting for mean-variance dependence. Methods: We analyzed actigraphy data from a subset of 72 participants with BD drawn from a larger longitudinal study, extracting 22 daily RAR features aggregated weekly as sample mean (MEAN) and within-week temporal variability computed as sample standard deviation (VAR). Variance-stabilizing transformations (Box-Cox or Yeo-Johnson) were applied to the entire study cohort to reduce mean-variance dependence. Associations with mood episodes and remission (mania: n=34; depression: n=58 annotated participants) were evaluated using generalized linear mixed-effects models with a logistic link function, including univariate (MEAN or VAR) and multivariate (MEAN+VAR) specifications, assessed by likelihood-based metrics and the area under the receiver operating characteristic curve (AUC). Results: Transformations reduced mean-absolute correlations from 0.43 to below 0.06. Temporal variability remained significantly associated with clinical state for 11/22 RAR features in mania and 16/22 features in depression, with all significant associations remaining after false discovery rate correction (p<0.05). Joint models showed modest incremental gains (AUC 3%-4% overall; up to 12% in mania, 7% in depression), with absolute performance remaining limited (AUC 0.50-0.66). In both mania and depression, nearly all significant variability-based regressors contributed incremental information beyond mean-based models. Only sleep duration and activity changes around wake time (+-1 hour), did not improve discrimination between mania and remission. Conclusions: Temporal variability in RAR features can be considered a standalone state marker of mood episodes not captured by mean activity. We found it to be more consistently associated with depression than mania. Its incremental discriminative contribution is modest, suggesting greater utility within multivariate or multimodal frameworks.

Background: Oral care is a critical nursing intervention for mechanically ventilated patients in intensive care units because it reduces oral microbial colonization and contributes to the prevention of ventilator-associated pneumonia, a major cause of morbidity, prolonged hospitalization, and increased healthcare costs among critically ill patients. Despite the availability of evidence-based guidelines, gaps in nurses knowledge and inconsistent implementation of oral care practices remain challenges, particularly in low- and middle-income settings. Objective: This study aimed to assess the level of knowledge among intensive care unit nurses regarding oral care for mechanically ventilated patients at Tenwek Hospital, Kenya, determine factors influencing knowledge, and examine the relationship between nurses knowledge and selected demographic and professional characteristics. Methods: An analytical cross-sectional study design was employed. The study was conducted among intensive care unit nurses at Tenwek Hospital. A sample of 38 nurses was selected from a target population of 60 intensive care unit nurses using simple random sampling. Data were collected using a structured self-administered questionnaire assessing socio-demographic characteristics, knowledge of evidence-based oral care practices, awareness of guidelines, and factors influencing knowledge. Data were analyzed using descriptive statistics, chi-square tests, Pearson correlation, and logistic regression, with statistical significance set at probability value less than 0.05. Results: Thirty-five nurses participated, yielding a response rate of 92.1 percent. The findings demonstrated generally high knowledge levels regarding oral care practices for mechanically ventilated patients. Most nurses correctly identified recommended oral care frequency (88.6 percent), oral assessment before care (91.4 percent), suctioning before and after oral care (94.3 percent), and the role of oral care in preventing ventilator-associated pneumonia (97.1 percent). However, knowledge gaps were identified in areas such as toothbrushing practices and documentation of oral care procedures. Training, continuing professional education, availability of guidelines, workload, resources, supervision, and teamwork were identified as important factors influencing knowledge. The findings revealed that professional qualification (probability value equal to 0.02), intensive care unit experience (probability value equal to 0.030) and formal oral care training (probability value equal to 0.021) were significantly associated with nurses knowledge of oral care practices. Conclusion: Intensive care unit nurses at Tenwek Hospital demonstrated adequate knowledge of evidence-based oral care practices for mechanically ventilated patients. However, targeted educational interventions are required to address identified gaps and strengthen standardized oral care delivery. Continuous professional development, consistent use of guidelines, supportive supervision, and adequate resource provision are recommended to enhance patient safety and reduce preventable intensive care unit complications such as ventilator-associated pneumonia. Importance: Patients who achieve remission from major depressive disorder (MDD) often face a preference-sensitive decision between continued antidepressant maintenance and discontinuation with active monitoring. Quantifying the tradeoff between depression burden and long-term medication exposure may support more individualized shared decision-making. Objective: To quantify tradeoffs between continuous antidepressant maintenance and active monitoring after MDD remission, and to identify preference thresholds favoring each strategy across relapse-risk strata. Design: Individual-level decision-analytic health-state transition model calibrated to randomized maintenance-discontinuation trials and a longitudinal first depressive episode cohort, with a 5-year time horizon. Setting: Outpatient clinical decision after completion of an 8-month continuation phase following remission from MDD. Participants: Adults in remission from MDD, represented across 4 clinically anchored relapse-risk strata ranging from very low risk after a first mild episode to high risk after highly recurrent depression. Exposures: Continuous antidepressant maintenance vs discontinuation with active monitoring and antidepressant restart after detected relapse. Main Outcomes and Measures: Severity-weighted depression-months, antidepressant medication-years, medication-years per depression-month averted, and net benefit across preference thresholds defined as the maximum additional medication-years a patient would be willing to accept to avert 1 depression-month. Results: Continuous maintenance reduced depression burden but required substantially more medication exposure, with efficiency strongly dependent on relapse risk. Medication-years per depression-month averted ranged from 11.8 (95% uncertainty interval [UI], 7.8-19.6) in the very low-risk group to 1.5 (95% UI, 0.8-3.0) in the high-risk group. At a preference threshold of 3 medication-years per depression-month averted, maintenance was preferred for moderate- and high-risk patients; at a threshold of 2, only for high-risk patients; and at a threshold of 1, for no risk group. Conclusions and Relevance: In this decision-analytic model, the value of continuous antidepressant maintenance depended strongly on baseline relapse risk and patient preferences regarding long-term medication exposure. These findings provide a quantitative framework for shared decision-making about antidepressant maintenance after remission from MDD.

Objective: Working memory (WM) deficits are a co-occurring feature to numerous neuropsychiatric disorders, particularly attention-deficit/hyperactivity disorder (ADHD), and there remain no treatments that directly target WM. The coupling between the phase of theta band activity and amplitude of gamma band activity (i.e., TGC) is an established neural correlate of WM. However, no studies have examined WM-related TGC in ADHD or whether neuromodulation can modulate these oscillatory dynamics in youth. This set of studies examined the effects of intermittent theta burst stimulation (iTBS) to the left dorsolateral prefrontal cortex (DLPFC) and left posterior parietal cortex (PPC) on TGC in youth with ADHD. Methods: In two randomized, double-blind, sham-controlled crossover trials, adolescents with ADHD and clinically significant parent-reported WM symptoms first completed a single-session study comparing DLPFC versus PPC iTBS targeting (n = 47) and then a multi-session clinical trial comparing 10 sessions of active versus sham left DLPFC iTBS (n = 29). Participants completed a computerized visuospatial Sternberg WM task with concurrent electroencephalography (EEG) before and after the single sessions, as well as at baseline, midway through treatment, and approximately 24 hours after the final session within the multi-session trial. Phase-amplitude coupling between theta phase and gamma amplitude was quantified using the Kullback Leibler modulation index at frontoparietal electrodes. Linear mixed-effects models examined treatment effects and associations between change in TGC and WM status (including accuracy, reaction time, and clinical symptoms). Results: Across participants, lower TGC was associated with lower symptoms and better WM performance, including higher accuracy, faster and more consistent RT. Active iTBS increased frontoparietal TGC relative to sham stimulation, with effects observed both acutely after a single session and ~24 hours after multiple sessions. DLPFC-targeted iTBS increased TGC, whereas PPC-iTBS had no measurable effect. Change in TGC was associated with change in WM, such that a decrease in TGC was associated with faster RT and decreased RT variability. Higher baseline TGC was associated with greater improvement in WM. Active iTBS decoupled the TGC-WM association observed during sham iTBS, and greater electric field intensity of iTBS was associated with greater improvement in WM accuracy and greater decrease in TGC. Conclusions: Active iTBS to the left DLPFC modulated WM-related TGC in youth with ADHD. These findings provide preliminary evidence that neuromodulation may improve WW by modifying oscillatory dynamics within frontoparietal networks. Larger clinical trials with higher stimulation doses are needed to determine whether targeting oscillatory coupling represents a potential therapeutic strategy for WM deficits.

The emotional content of suicide notes is typically examined using categorical coding, where each labeled passage is treated in isolation from its surrounding language. In contrast, dimensional models of psychopathology propose that affective content varies along continuous gradients. We evaluated this proposition directly. Excerpts from 884 annotated suicide notes were embedded in a semantic space defined solely by their linguistic properties, and we investigated whether human-assigned emotion labels changed smoothly across this space. They did: affective tone showed clear spatial autocorrelation (Moran's $I = 0.18$, $z = 19.68$, $p < 0.001$), an effect that replicated across three different encoders and remained after removing all within-note dependencies. Emotions occupied recognizable yet overlapping regions rather than forming distinct clusters and varied substantially in how tightly they were concentrated: love and hopelessness appeared with similar frequency, but love was far more localized ($z = 15.7$ versus $10.8$). Among all emotions, hopelessness was the most linguistically diffuse, implying that a single categorical label is capturing multiple, qualitatively different manifestations of suicidal distress.

Objective: Polygenic risk scores (PRS) for posttraumatic stress disorder (PTSD) often account for a low amount of variance. Ancestry-related differences in PRS scale and variance limit cross-group comparisons. This methodological challenge further complicates gene-by-environment (GxE) analyses, given that socioenvironmental exposures are inequitably distributed across ethnoracial groups. We constructed an ancestry-calibrated polygenic risk score (AC-PRS) for PTSD in the largest longitudinal study of trauma survivors to date and investigated GxE interactions. Method: Recent trauma survivors (N=1,801) provided a blood specimen for genotyping. Six PTSD trajectories were previously identified from PTSD Checklist for DSM-5 (PCL-5) scores at 2-weeks, 8-weeks, 3-months, and 6-months post-trauma. Greenspace (normalized difference vegetation index [NDVI) and socioeconomic disadvantage (area deprivation index [ADI]) were derived from residential addresses. Logistic regressions examined interactions between newly developed AC-PRS and neighborhood factors on trajectories after adjusting for sociodemographic and trauma-related covariates. Secondary linear models considered GxE interactions on 6-month PCL-5 scores. Results: AC-PRS performed well across ethnoracial groups, explaining significant variability in PTSD trajectories (R2=.053). ADI moderated the association between AC-PRS and the likelihood of assignment in a high nonremitting trajectory of PTSD symptoms and severity of symptoms at 6-months (ps < .05). There were no NDVI x AC-PRS interactions in any models. Conclusions: AC-PRS captures genetic risk for PTSD in admixed trauma survivors, demonstrating good discrimination between nonremitting and resilient courses of PTSD. However, neighborhood disadvantage may modify utility of PRS for PTSD, warranting careful consideration when applying these scores across contexts.
